About this trial
This trial looks at how to help people with early-stage breast cancer take their hormone therapy and heart medications regularly. The goal is to see if a special program can improve adherence over 24 weeks. Researchers will check how well participants stick to their medication plans using surveys and pharmacy records.

Who can join
This study is for men and women aged 18 to 99. You can join if you have stage I-III breast cancer and are on hormone therapy. You must also be prescribed at least one heart medication. Those who do not speak English or Spanish or have cognitive issues cannot participate. The trial team will confirm if you are eligible.

What happens in the study
Participants will be randomly assigned to one of two groups. One group will receive a special program to help them remember to take their medications. This includes a session with a pharmacist and training on using a reminder app. The other group will receive usual care from their doctors. Everyone's progress will be tracked for 24 weeks.
